Consent - Please Read

Dear Parents and Caregivers

The following form is to request approval for your child to access online third-party services that support their learning in the classroom. Third-party vendors provide the services and are not managed by the Department of Education. Each of the third-party services has been assessed by the Department as approved for use in WA public schools.

To implement them, we may need to provide some personal information about your child, and we are seeking your consent to do so. The information provided to third parties varies between providers. The personal information provided is listed below for each service for your convenience.

When providing consent, it is important that you are aware of what type of information we will be providing, how it will be used, and where it will be stored. If you do not provide consent, your child will not have access to third-party online services.

The online services provide our students with significant educational benefits. The school will contact you to discuss your reasons should you not consent or fail to respond. If you do not consent, your child may receive an alternative education program that does not make use of these services.

The consent you provide for your child will apply to each of the mentioned third-party services until the end of their schooling at Maidens Park Primary School. We may ask you for consent to other third-party services in the future.
