Families are invited to complete an application for enrolment form and return it to the school office.
When you enrol your child at our school, please provide the following:
Birth Certificate
A statement of Immunisation from MYGOV (Medicare) (if you are unable to provide this, please complete the Request for immunisation support from the Department of Health form)
Proof of address (rent/lease agreement, electricity account, etc)
Court Order (if applicable)
If your child is not an Australian citizen, you will need to provide evidence of the visa subclass number and status that you or your child holds. Visa holder information here.
Enrolment applications and supporting documents can be handed directly to the school office or sent to MaidensPark.PS@education.wa.edu.au
